Flat Tops Counseling provides substance use evaluations for individuals and referral situations. Evaluations are based on available information and clinical judgment, not on a promised outcome.

What an evaluation may include

  • Reason for evaluation and referral context
  • Substance use history and current pattern
  • Alcohol and drug consequences
  • Mental health and safety screening
  • Medical, withdrawal, overdose, or detox concerns
  • Legal, probation, employer, or attorney-related concerns
  • Prior treatment and recovery supports
  • Diagnostic impression and recommendations

Important acceptance notice

Flat Tops Counseling does not guarantee that an evaluation, report, recommendation, or letter will be accepted by a court, probation department, employer, attorney, DMV, licensing board, treatment provider, insurance plan, or other third party.

Clients are responsible for confirming requirements with the referral source before scheduling, unless Flat Tops Counseling has directly confirmed those requirements.

Reports and releases

Information is not released to courts, probation, attorneys, employers, family members, medical providers, or other parties without a valid Release of Information unless disclosure is required or permitted by law.

Recommendations may include no treatment, outpatient counseling, higher level of care, medical or detox referral, mental health referral, peer support, or other appropriate referrals.
