Flat Tops Counseling, LLC does not currently accept or bill insurance directly. Payment is due at the time of service. A superbill may be provided for clients who wish to submit to their insurance plan for possible out-of-network reimbursement. Reimbursement is not guaranteed.
Documentation and payment notes
Standard documentation is included when appropriate. Additional, extensive, rushed, repeated, legally contested, or specially requested documentation may require a separate fee, which will be discussed in advance.
Accepted payment methods may include cash or check. Confirm current payment options when scheduling.
